Obtain a Script from Your Doctor
In the state of Texas, Physical Therapy Services require a referral from your doctor.
Contact our Office
Once you have obtained a script from your doctor, contact our office at (512)-285-4406 and fill out our new patient forms. The Patient Care Coordinator will get your insurance information and verify your benefits and communicate them to you.
What to bring to first session?
Bring your driver's license, insurance card, medication list, and any imaging reports you have from your doctor.
What will my first session consist of?
The therapist will complete an evaluative examination and determine a plan of care with you. You and the therapist will set goals for your time in physical therapy. Please wear comfortable clothing for your initial evaluation.
